Treatment for teeth grinding, jaw clenching and gummy smiles

We start with the cause. Your dentist examines your teeth, jaw muscles, jaw joint and smile, then talks you through the options, from a custom night guard to muscle-relaxing (botulinum toxin) injections. Injections are carried out only by Dr Celeste Ng and Dr Celeste Ling, accredited by the Singapore Dental Council. Jurong West, Clementi and Tanjong Pagar (Vera Dental Implants).

Key Takeaway

Teeth grinding, jaw clenching and jaw-muscle pain are assessed at a one-to-one consultation. Depending on the cause, options include a custom night guard, advice and care for the jaw joint, and botulinum toxin injections to relax an overactive jaw muscle. A gummy smile caused by an active upper lip may be treated with a small injection; other causes need gum or orthodontic treatment. Injections are carried out only by Dr Celeste Ng and Dr Celeste Ling, accredited by the Singapore Dental Council. Whether treatment suits you, and the dose, are decided at your consultation.

How Is Treatment Decided at Trust Dental?

There are no set packages. At your consultation, your dentist looks at what is causing your symptoms: worn or cracked teeth, tender jaw muscles, clicking or pain at the jaw joint, or how your upper lip moves when you smile.

She then explains the options that suit your situation, including what each can and cannot do and the possible side effects. If an injection is appropriate, the dose and the fee are discussed with you at that consultation, before any treatment. Botulinum toxin injection is not claimable under Medisave or CHAS.

CONDITIONS WE TREAT

What we assess and treat

Treatment starts with the problem, not the product. These are the jaw and smile concerns we see most often.

Teeth grinding (bruxism)

  • Signs: worn, chipped or sensitive teeth, and a tired jaw in the morning
  • A custom night guard is the main way to protect your teeth from wear
  • For strong grinding, injections into the jaw muscle may reduce the force; they do not cure bruxism
  • Read: treatment options for grinding & clenching

Jaw clenching & jaw-muscle pain

  • Aching or tight jaw muscles, tension around the temples, or a firm, enlarged muscle at the angle of the jaw (masseter)
  • We check whether the muscle, the jaw joint, a tooth or a combination is involved
  • Options include a night guard, jaw care advice and, if suitable, injections to relax the overactive muscle
  • Read: masseter injections for jaw clenching

Jaw joint clicking or pain (TMD)

  • Clicking, locking or pain in front of the ear can come from the jaw joint itself
  • We examine the joint, muscles and bite before suggesting anything
  • Care may include a splint, self-care advice or referral to a specialist when needed
  • Read: night guards and bruxism

Gummy smile

  • A gummy smile can come from lip movement, gum height, tooth position or the jaw
  • When an overactive upper lip is the cause, a small injection may relax the lip-lifting muscles
  • Other causes need gum reshaping or orthodontic treatment instead

What happens at your consultation

Assessment first. Treatment only if it suits you.

1

Assessment

Your dentist asks about your symptoms and medical history, checks your teeth for signs of grinding, feels the jaw muscles and joint, and looks at how your lips move when you smile.

2

Options and consent

She explains the options that suit you, such as a night guard, jaw care advice or a muscle-relaxing injection, and the possible side effects. If an injection is suitable, she decides the dose at this stage.

3

Treatment, if suitable

For an injection, the area is cleaned and marked. A very fine needle is used, and each injection takes seconds.

4

Aftercare

For the rest of the day: don't rub or massage the area, avoid lying flat for a few hours, and skip strenuous exercise and saunas.

5

Review

The effect builds over several days, with the fuller effect usually at around two weeks. Your dentist will advise when to review.

IS IT RIGHT FOR YOU?

Safety and suitability

Possible side effects of injections (usually temporary)

  • Tenderness, swelling or a small bruise where the needle went in
  • Headache
  • Chewing tougher food may feel tiring for a while after jaw-muscle injections
  • A temporarily uneven smile after gummy smile injections

Injections are not suitable if you

  • Are pregnant or breastfeeding
  • Have a neuromuscular condition such as myasthenia gravis
  • Have an infection at the injection site
  • Have had an allergic reaction to botulinum toxin

Please tell your dentist about all the medicines you take. The effect of an injection is temporary and varies from person to person.

FAQ

Common questions about grinding, clenching & injections

Do you have fixed prices or packages for botulinum toxin injection?+

No. We do not offer packages or promotions. Whether treatment suits you, and the dose you need, are decided by your dentist at a one-to-one consultation, where the fee is explained before any treatment. Botulinum toxin injection is not claimable under Medisave or CHAS.

Who performs the injections?+

Dr Celeste Ng and Dr Celeste Ling, who are both accredited by the Singapore Dental Council to perform botulinum toxin injection. In Singapore, a dentist must hold this accreditation before offering the treatment.

What is the first step for teeth grinding or jaw clenching?+

A consultation. Your dentist checks your teeth for wear, the jaw muscles and the jaw joint, and asks about your symptoms. For most people, a custom night guard is the first step to protect the teeth. Other options are discussed if symptoms continue.

Can botulinum toxin help with teeth grinding and jaw clenching?+

Injections into the masseter (the large chewing muscle at the angle of the jaw) relax the muscle, which can reduce the force of clenching and grinding and ease jaw-muscle tension. It does not cure bruxism, and a night guard is still the main way to protect your teeth from wear. Your dentist will assess whether it suits you.

How long does the effect of an injection last?+

The effect is temporary and wears off gradually as the muscle recovers, typically over several months. How long it lasts varies from person to person, so repeat treatment is needed to maintain the effect.

When will I notice a difference?+

The effect builds up over several days, with the fuller effect usually seen at around two weeks.

Does it hurt?+

A very fine needle is used and each injection takes seconds. Most people describe a brief pinch. Mild tenderness or a small bruise at the injection site can occur and usually settles within days.

What are the possible side effects?+

Common, temporary effects include tenderness, swelling or bruising at the injection site and headache. Jaw-muscle injections can make chewing tougher foods feel tiring for a while. Gummy smile injections can occasionally make the smile temporarily uneven. Your dentist will go through the risks at your consultation.

Who should not have botulinum toxin injection?+

It is not suitable if you are pregnant or breastfeeding, have a neuromuscular condition such as myasthenia gravis, have an infection at the injection site, or have had an allergic reaction to botulinum toxin before. Tell your dentist about all medicines you take.

Is there any downtime?+

Most people return to normal activities straight away. For the rest of the day, avoid rubbing or massaging the treated area, lying flat for a few hours, strenuous exercise and saunas. Your dentist will give you aftercare instructions.
